Here at Life Connections, we are passionate about those we serve. Since our inception in 2004, our vision has been to provide our clients with exceptional service that promotes dignity, self-esteem, and self-reliance.
To achieve this mission, we focus on three key components: Communication, Dignity, and Personalization.
Our Services
Community Living Service
Our staff completely believes each of our residents can live full, independent lives with the right level of support for their activities of daily living. This belief shows in how our staff support our residents every day.
Community Support Services
With services offered from 8 am to 4 pm Monday through Thursday, Life Connections helps individuals access the community. Our staff members actively work to ensure every trip into the community is a positive experience.
Adult Day Services
Our adult day services all focus on engaging our clients in daily life, whether it’s their daily living activities, doctor’s appointments, or activities within our facilities or in the community. We find these services help our clients maintain their independence where possible.
